The Effect of a Community Health Worker-Based Home Visit Healthcare Model on Improving the Quality of Life of Older Adults with Chronic Diseases in Coastal Areas

Older adults in coastal areas often face limited access to healthcare services due to geographical, infrastructural, and economic barriers. Chronic diseases among older adults may further reduce their quality of life due to inadequate health monitoring and limited access to continuous care. This study aimed to analyze the effect of cadre-based home visit healthcare services on improving the quality of life of older adults with chronic diseases in coastal areas. A quasi-experimental design with a pretest–posttest approach involving intervention and control groups was conducted from April to July 2026 at Puskesmas Air Salobar and Puskesmas Latuhalat, Ambon City, Indonesia. A total of 70 older adults aged >60 years with chronic diseases were selected using purposive sampling. Data were collected using the WHOQOL-BREF questionnaire and analyzed using the Wilcoxon signed-rank test and the Mann–Whitney U test. The results showed that the intervention group experienced a significant increase in quality-of-life scores from 79 (53–110) at pretest to 99 (60–117) at posttest (Z = -4.358; p < 0.001). The control group also showed improvement from 79 (59–104) to 82 (61–108) (Z = -3.126; p = 0.002); however, the improvement was lower than that observed in the intervention group. These findings indicate that cadre-based home visit healthcare services effectively improve the quality of life of older adults with chronic diseases. Strengthening the role of health cadres in community-based primary healthcare can provide a sustainable approach to overcoming healthcare access barriers in coastal areas.
